LHC takes notice of lack of renal facilities at public hospitals - The Nation PDF Print

LAHORE - The Lahore High Court on Monday sought reply from Punjab health department in a petition challenging lack of kidney treatment facilities at public hospitals in the province.
Advocate Farooq Badar filed a public interest petition submitting that else than Mayo Hospital of Lahore, there is no adequate facility for kidney patients in government hospitals of the province.
The number of kidney patients has drastically increased in recent years but the government has failed to ensure proper treatment for them, he added.
The petitioner pointed out that the patients are forced to go abroad for the treatment due to poor health facility at local hospitals.
He asked the court to order the government to provide proper treatment for kidney patients in the province.
The court has sought reply from the government within ten days.

Dehydration may lead to acute renal failure in kids, say experts - Times of India PDF Print
AURANGABAD: With the temperature rising, a host of health problems are affecting children in the city, beginning from minor heat ailments, urine infection and diarrhoea to jaundice and even chicken pox. Hospitals have also been witnessing cases of acute renal problems, which can be due to severe dehydration due to summer season.

Over the past few days, the mercury has been soaring in the city with temperatures reported above marginally normal. On Monday, the city recorded a maximum temperature of 40.4 degrees celsius and a minimum of 25.4 degrees celsius, both were one degree above normal.

City-based paediatric nephrologist Pankaj Bhansali said, "Every year, our hospital reports around 15 newborn patients suffering from renal problems in summer. Dehydration plays a major role in causing acute renal failure in neonates."

Neonatologist L S Deshmukh at the Government medical College and Hospital (GMCH) also said that complications of dehydration include renal problems, jaundice, seizures and brain damage. "The Neo-natal Intensive Care Unit (NICU) is reporting cases of jaundice due to dehydration. One in four newborns suffer from jaundice," he said.

Bhansali said as many as eight neonates suffering from renal problems accompanied with fever, poor sucking and decreased urine output were admitted between April and May 2015. "Renal failure during summer is generally due to dehydration and if treated on time, the children can recover very well. But, if treatment is delayed, it effects brain and kidney," he said.

"Such cases occur when the baby's feed is inadequate leading to water dehydration. In some cases, post delivery mothers either have decreased milk production or have nipple-related abnormalities that create feeding difficulty and result in inadequate fluid and caloric intake in their infants. Especially, in summer if the baby is poorly fed and is dehydrated the child's activities too reduce further," warned Bhansali.

He said, "To address such cases, timely feeding in summer every 2-3 hours is necessary. First-time mothers should not be discharged from hospital until proper breastfeeding technique is observed. Mothers with identifiable problems should be referred promptly to lactation management specialists and close follow-up should be maintained following discharge."

He also advised that the urine output of the neonates should be monitored closely during summer. "The baby should pass urine at least 6-8 times a day and the lactating mothers should drink adequate milk and water," he said.

While describing other summer ailments, he said urine infection and heatstroke are the other worrisome health problems reported among children. "Repeated urine infection is caused due to less water intake and more water loss from the body. Parents should make sure that children urinate often instead of controlling it," added Bhansali.

Deshmukh said, "Dehydration fever is another the common ailment observed when the temperature rises. Jaundice cases too go up due to dehydration as bilirubin, which needs adequate water to be cleared from the body. If the baby gets less water, it can lead to jaundice."

"Preventive steps such as adequate feeding, maintaining room temperature between 26-28 degrees celsius is required to avoid complications. If the child is not treated on time, it can also prove fatal," cautioned Deshmukh.

"Cases of heatstroke are usually common when the mercury reaches 40 degrees celsius," said former Indian Paediatric Association (IAP), Aurangabad chapter president Prashant Jadhav.

He advised that children should drink enough fluid before indulging in sports activity and prevent playing in the sun. "There should be 30% more intake of water during the summer season. Juices should preferably be homemade," he added.

  Prevalence and clinical impact of magnesium disorders in end-stage renal disease: a protocol for a systematic review Magnesium plays a key role in maintaining internal homeostasis through actions in the musculoskeletal, nervous, endocrine and cellular messenger systems. Renal excretion is the major route of magnesium elimination from the body. A positive magnesium balance would be expected in renal failure. However, a compensatory decrease in tubular reabsorption is expected to operate to maintain adequate urinary magnesium excretion even when glomerular filtration rate is very low.Patients with end-stage renal disease and those on dialysis have impaired regulatory mechanisms, predisposing them to disturbances in magnesium levels. The effects of high or low magnesium can have deleterious health outcomes, which impact on the co-morbidities and outcomes of chronic renal disease. This systematic review aims to determine the prevalence and clinical outcomes of magnesium disorders in end-stage renal disease.Methods/designWe will undertake a comprehensive search of various databases, MEDLINE, PubMED, EMBASE, Cochrane Library, Cochrane Collaboration, CIHNAL (Ebsco), Web of Science and Google Scholar, for observational studies and clinical trials on magnesium disorders in end-stage renal disease using key terms to identify papers for inclusion. Paper selection and data extraction (where appropriate) will be performed in duplicate on socio-demographic characteristics of participants, diagnosis of end-stage renal disease, magnesium levels, prevalence and clinical outcomes. An assessment of quality will be performed using a modified Newcastle-Ottawa Scale (NOS), including identification of any bias, which may influence findings. Data will be pooled together according to whether the studies were on pre-dialysis, hemodialysis or peritoneal dialysis participants. References from individual papers will also be screened as appropriate. Fort McMurray, ESRD preparing amid Alberta wildfires - Fort McMurray Today PDF Print

With wildfires burning on nearly every border, the Fort McMurray wildfire management area of the provincial Environment and Sustainable Resource Development department is ramping-up preparations in their own district.

The local team received an additional five firefighters from Ontario, as part of the 126 firefighters who arrived in Alberta from Ontario on Friday.

According to Robyn Haugen, a spokesperson for the Fort McMurray area, one senior firefighter has been dispatched to Fort Chipewyan, while the other four are stationed in Fort McMurray.

“We are pre-positioned to be able to manage anything that comes up,” she says. “Obviously it is still hot and dry out, and we're not expecting a whole bunch of relief in the forecasted future. So we are prepared.”

As of Sunday afternoon, fires were burning in the neighbouring Lac La Biche and Slave Lake areas, as well as across the border in Saskatchewan. The fire near Cold Lake, Alta. had grown to 10,000 hectares and forced the evacuation of all staff at Cenovus' Foster Creek and Athabasca Gas operations.

“Obviously, we're keeping an eye on everything that's going on,” Haugen says. “And we are prepositioned, and we're manned-up.”

The department has been flying routine helicopter air patrols throughout most of Wood Buffalo during the fire season. They're also relying on tips that come in through the 310-FIRE line to spot potential wildfires.

Much of Northern Alberta has been placed under an elevated fire hazard warning. Fort McMurray has been placed under an extreme wildfire hazard warning, and Haugen has said that officials are considering instituting a fire restriction for the provincially controlled areas.

A fire restriction would ban campfires and open fires in “backcountry or random camping areas.”

“We're asking people to absolutely make sure their campfires are fully extinguished,” Haugen says. “And to make sure if they're quadding or using their Off Highway Vehicles out in the forested areas that they're making sure they knock off the hot debris and they look behind them for any smouldering”
